About Clare Housing

The long-term goal of Clare Housing is to develop a continuum of care that will meet the housing and service needs of people living with HIV/AIDS today and in the future.

Responsive

Clare Housing's care and housing models respond to the current needs of people living with HIV, creating individualized support of each individual's personal journey with HIV.

Flexible

Our housing and care models are flexible and forward-thinking; allowing us to adjust care or service delivery methods as our client's needs change or the demographics of people living with HIV changes.

Collaborative

There are complex medical, psycho-social, and financial difficulties inherent in HIV/AIDS. In order to secure the best success for each individual, staff members work with our residents and their care teams (case managers, home care nurse, doctors, family and friends) to develop, evaluate and update personal care plans for our residents.
